- Aadhaar Number: Fill 12 digit Aadhaar Number.
- Name of Applicant: Fill your name as per Aadhaar Card.
- Social Category: Select your social category from the options available.
- Gender: Select your gender from the list.
- Physically Handicapped: Select the status from available options.
- Name of Enterprise/ Business: Enter the business name which you want to print on MSME Certificate.
- Type of Organization: Select your organization from the list.
- PAN Number: Enter your 10 digits PAN Number.
- GST Number: Enter your GSTIN Number (if any).
- Location of Plant: Please enter the address where your plant is situated.
- Office Address: Please provide your office address if the address is different from the plant location.
- Select Your State: Kindly select your state in this field.
- Mobile Number: Enter your 10 digit mobile number.
- Email ID: Kindly enter your email id.
- Date of Commencement of Business: Fill the date on which you started your business. This will be printed on the MSME Registration Certificate.
- Bank Account Number: Fill your bank account number in this field. (Any Saving or Current).
- Bank IFSC: Enter your Bank IFSC number. You can check the same on your passbook or cheque book.
- Main Business Activity of Enterprise: Select the Main Business activity from the given options.
- NIC 2 Digit Code: Kindly select the 2 Digit NIC Code from the available options considering your business activity.
- Additional details about Business: Fill Additional details about business. (For example – Service, manufacturing of Food Products, Software development, Etc.).
- Number of employees: Enter the number employees working in your plant.
- Investment in Plant & Machinery / Equipment: Here you have to enter the investment made by you in your Plant or Machines.
- Attachment: Kindly attach your Aadhaar Card (jpg, png file <500 kb).

Is GST Registration compulsory for me?

Do I need GST number to sell on online portals?
Yes. If you are listing taxable goods, GST details are required to sell online. You need to provide GST number to Amazon at the time of registration.
Note that if you start selling any taxable goods you need to register for GST as per GST laws and provide your GST number to Amazon.
How can I get my GST number online?
To obtain this, you need a valid mobile number (an India number), email address and PAN (Permanent Account Number) for the business, Proof of Business Place.
